Small input formatting bug fixes and new BIN numbers

This releases fixes a number of small bugs around input formatting (e.g. see #187 and #215) and adds support for some newer credit card BIN numbers.

Fix 'jumping cursor' issue and improve support for IMEs

This release resolves the issue where the cursor would jump to the end of the input upon any change to the value. It also partially resolves contention with input method editors (IMEs), which provide input assistance for certain locales.

Support new MasterCard BIN ranges
