Digital plan for net neutrality

After hashing this out on the phone with Sara and Scott, here's where we're at:

December 4:
Launch call tool - advocacy.mozilla.org/net-neutrality as a reskin of changecopyright.org. There will be no modal.

December 14:
If the vote happens, we want to swap out the content of /net-neutrality to be the signup form (with some changed text, TBD. The donate modal will appear after someone submits the form. The call tool/about the situation page will still be present, but at different URLs.
