Probably, This is by design of GitBook. There are two needs.
Maybe, we need to provide some special block for this use-case.

Summary
In the case of README.md(a.k.a. root for
index.html
), the variables inside code block is not replaced and encapsulated inside a pre-code html tags. But in the case of included files, any book variables inside the code block is replaced with the varialbe value defined insidebook.json
