Rich Text: Bullets get confused and display both 1st and second level, cannot TAB/ShiftTAB #1459

Description

Somehow bullets got confused in the linked document: enter on second level created a 1st+2nd bullet mark (and acts like it's second level), a new enter crteates a proper 1st level.
TAB / Shift+TAB doesn't work anymore, indent/outdent buttons work, but the result is pretty random.

It can't seem to be fixable by in/outdenting.

May be related to #1111 ?

Steps to reproduce

Well I was creating a title, a second level title, a bullet point, then enter, then tab, started writing 2nd level, enter, then there was the 1st+2nd level indicator, but I cannot always reproduce it.

Expected behavior

Either 1st or 2nd, not both.

Which operating system are you using?

Linux/BSD/UNIX

What browsers are you seeing the problem on?

Mozilla Firefox

Extensions

Plenty, but I don't think they matter here.
