Ferry should replace course codes in evaluation questions with placeholder

Some historical evaluation questions explicitly mention the course code. We only store the question text once for each question code, not for every course, so users might see the question mentioning a different course. We should replace the course code with a placeholder like [course code] which can get replaced with the actual course code on the frontend.
