Current practice is for staff to check for current active/previous punishments and escalate based on that. If I am understanding, we should provide a prompt to extend a given punishment if a staff is suspending someone already suspended. Should also probably mention number of extensions, in case it's like a 3-suspend = ban policy.

Sure, I like the prompt idea. That's actually what I had in mind (a prompt that asks you to confirm extension of times punishment by either pressing the EXTEND button or OVERRIDE button).

I'm also okay with the idea of listing the number of times the punishment has been extended/overridden. I don't want to include any policy in the bot regarding number of extensions or whatever with a higher punishment because then we'll be asked to add some customizability regarding this, and well you know that's tedious; imo if it becomes more severe, staff can handle it themselves.
